The information below is used to assess whether an Independent Initiative Review fits the decision and what access it would require. Submitting the form does not book or begin an engagement.

Please include

  • One defined initiative
  • The decision and its deadline
  • The commitment being considered
  • A named owner and sponsor
  • The evidence and stakeholders available

Complete every field except those marked optional.

Describe the choice and any options that remain open.

Briefly describe the work, intended result, and people affected.

Give the owner's name and role.

This may be a budget, contract, hire, platform choice, acquisition, or public promise.

Give the sponsor's name and role, or write “None”.

List the business case, technical assessment, proposal, prototype, research, or operating data available.

Add any relevant constraints, sensitivities, or links.

I will reply with an assessment of fit and scope.

I will assess whether the work fits the practice, what information and stakeholder access it requires, and which engagement is proportionate.

I will also flag work that belongs with an implementation partner, specialist auditor, lawyer, transaction adviser, or another provider.
